Consider digital paper forms

You don’t need relational algebra in your software. You need a file locker to store forms in. NoSQL? Yeah, that works too. Any POSIX file system works by itself.

Allow the form to submitted even if the “required” fields are not filled in. Keep the partially filled form for your customer. They may come back later.
